Do I have to mail anything other than the form NYC-1127 ?

According to the instructions for Form NYC-1127, you are required to mail the following items:

A complete copy of New York State Income Tax Return, including all schedules
Wage and withholding statement (Form 1127.2)
Copy of federal Schedule A, if itemizing deductions
Agency verification, if claiming a line of duty injury deduction
