To correct an error on your Arizona Form 322 (Public School Tax Credit) after you have already submitted your taxes, you do not need to redo your entire original tax return from scratch, but you must file an amended tax return using Arizona Form 140X (Individual Amended Income Tax Return).

Because a wrong or missing Public School CTDS code is one of the most common processing errors flagged by the Arizona Department of Revenue (ADOR), filing an amendment ensures your tax credit is properly matched and not disallowed.

Step-by-Step Correction Process

  1. Wait for Processing: Do not file your amendment until your original return has been completely processed by the state.
  2. Obtain the Correct Code: Make sure you have the accurate 9-digit CTDS code (County, Type, District, and Site code) for the public school. You can look it up using the Arizona Department of Education School Search or the tax credit resource page on the ADOR web site.
  3. Fill Out a New Form 322: Complete a corrected version of Form 322 for the respective tax year, entering the proper 9-digit code into the required column without spaces or dashes (e.g., 012345678).
  4. Complete Form 140X: Fill out Arizona Form 140X for that specific tax year.
    • If the correction only changes the CTDS code and not the dollar amount of your tax credit or overall tax liability, your financial columns (Original Amount, Difference, and Amended Amount) will stay the same.
    • In Part 2 (Explanation of Changes), write a concise note such as: "Correcting an erroneous Public School CTDS Code on Form 322, Line 6."
  5. Submit the Amendment: Depending on the tax year and the software you originally used, you may be able to e-file the amendment. If your software does not support it or if it is for an older year that requires paper filing, print out Form 140X, attach your corrected Form 322, sign it, and mail it to the address listed on the form instructions.
