I just efiled my taxes and then received a 1099-R for a 401k rollover. It shows a zero taxable amount. Do I still need to file an amended return?

Yes, you should include the 1099-R form in your return because it was also sent to the IRS and they match 1099-R forms issued with forms filed with your return.
