Why am I being taxed on my qualified 401K Roth withdrawal in Pennsylvania? (I met both requirements of age and had account more than 5 years)

Code 7 in box 7 of the Form 1099-R is not taxable to Pennsylvania.  Choose "I'm eligible, plan eligible" when prompted to mark it as not taxable on the PA return.

If there is a code 7D, then it is taxable.
