That is something that you set up with your bank. But if you have the form 1099R in front of you then you can skip the import and enter the information yourself. On the "Let's get your tax info. We’ll help you find and import info from your bank or financial institution" screen scroll down and click on the "I'll type it in myself" button (see picture). Then you can enter the information yourself.
Make sure you import your Fidelity 1099R information. I typed mine in from the printed 1099Rs and the address was completely different from the imported one. The one I typed in caused the federal return to reject. I had typed everything exactly as printed. After I deleted the typed ones and imported them, the returns were accepted. The printed address did match the tax id number that is filed with the IRS.
