A Form 5498 is an informational form only. It is not entered anywhere on a tax return.

Where do I enter information from Form 5498?

It may be information only, but the TTax IRA Information Worksheet PartIV asks for the Year End Value of your Traditional IRAs. It further directs you to See Help which in turn guides one to obtain these values from Form 5498.
