taxpayer retirement distribution were entered on your federal tax. do i just put georgia taxes and if yes do i put my retirement or do i add my social security taxes were taken out of there also,

Total contribution is about 1k to retirement via my work. Do I need a separate form for deductions on that money, or is it included in box G-12 on my W2? Different? Same?
