There is a FATCA check box between BOX 11 & 12 on your 1099R form. Check your original to see if it checked and then check your entry in Turbo Tax. If it isn't checked on your original hard copy, uncheck this in your Turbo Tax return. Meanwhile please refer to this IRS link for further information about FATCA.
You say to look at box 11 or 12 for a box. I dont have a box 11, and box 12 is my state tax withholding amount on my 1099r form. So the answer doesn't help me. What does FATC mean?
The checkmark will be listed in box 12 of your 2019 1099-R. Please view my screenshot below for reference. The acronym FATCA stands for the Foreign Account Tax Compliance Act. Taxpayers who file a return that includes foreign assets with an aggregate value of more than $50,000 are required to report their assets on form 8938. If your 1099-R form has a checkmark in box 12, then you meet this filing requirement and must attach Form 8398 to your tax return. If there is no checkmark, then you are not required to do so.

Thanks so much for sending a picture. I have 2 1099R forms from 2 different places and they look different. One form looks exactly like the one you posted, and I can see the Box 12. The other form is different. It shows 1099R 2019 but does not have box 10 or 11 and box 12 is where the "state tax withheld" amount is. So I will go with what information I have, which is zero.
Thank you for taking the time out to help.
