Contact the financial institution that issued the retirement money and request that they send you a 1099R, or figure out how to get to it on their web site. Many banks and financial institutions have sites where you can get to it online.

To enter your retirement income, Go to Federal> Wages and Income>Retirement Plans and Social Security>IRA 401 k) Pension Plan Withdrawals to enter your 1099R.
