You can create a substitute form 1099-R if you are sure that you are required to have one and that you are required to report your retirement transaction. However, I do recommend that you contact your retirement administrator to verify why the form 1099-R is not issued... just to make sure that you should in fact have one.
In 2024 I have removed excess 2023 contributions plus earnings from ROTH IRA. I do not have a 1099-R as the this removal was in 2024.

Your earnings must be reported as taxable income for the year the excess contribution was made, so you’ll still receive a 1099-R to report these earnings on your tax return. You will need to amend your 2022 return to report the earnings. Here is how to report.

Now you need to create a substitute 1099R.
You will receive a 1099R for this event next year but you do not need to report this since you are reporting this for year 2023.
