W2 confusion

Hi why is box one and two on my W2 blank and box 3 through 6 have amounts and the rest of the boxes are blank?

    Is this a W-2C(Corrected W-2) that is showing the changes to the original W-2?

    Yes but the wage is nowhere near what I made basically they took a couple dollars from box 3 & 5 and put it in box 1.
