We live in Pennsylvania.  We are both retired receiving income from social security, pensions, annuity, and traditional IRAs.  Pennsylvania does collect taxes from any of these income sources.  Do I need to fill out and file state income tax forms?

No, if you do not have any other income that would be taxable in Pennsylvania (such as dividends and interest) you are not required to file a state income tax return.
