If it's asking you to pick from listed institutions in a menu, that's just for importing.   Not all institutions participate.   It's easier to enter it manually. 

I'll assume you are already in the 1099-R interview since you didn't say otherwise.  Scroll down on that screen where it asks for an institution, and near the bottom indicate that you want to enter it another way.   Then on another screen say you will type it in yourself.   Then you'll get boxes where you can manually enter the name of the Payer and the other boxes as shown on your Form 1099-R.
