Which Form 5329 is used for a late RMD

My mom forgot to take out enough for her RMD by 12/31/2021.    Do I use Form 5329 for 2020 or 2021 to report and ask for an acceptation to the tax?

If  a 2021 RMD that was missed then a 2021 5329.

But what does 2020 have to do with it if it is a 2021 RMD?
