No, it is not, but you should still enter the information from the 1099-R onto your tax return.
It's generally only a qualified distribution if you have reached age 59½ and on the date of the distribution it had been at least 5 years since the beginning of the year for which you made your first Roth IRA contribution. Otherwise the distribution is not a qualified distribution and the ordering rules for Roth IRA distributions apply.
