The 1099R death benefit shows in 2a Taxable Amount as unknown. I don't know what to put on the tax return for taxable amount.

Please call the issuer of the 1099-R -- they should be able to tell you the basis, even if they have checked the  box "taxable amount not determeined".  Otherwise the gross distribution is the taxable amount
