What is your question? You need to enter your pension 1099R on your tax return. It may be taxable on your federal return depending on how much other income you received. Some states may tax it too.

To enter your retirement income, Go to Federal> Wages and Income>Retirement Plans and Social Security>IRA 401 k) Pension Plan Withdrawals to enter your 1099R.
