On my form 1099-R, Turbo is asking for info in boxes I do not have. What do I do??

You should enter the 1099-R as it appears and leave those boxes blank. The IRS will match your 1099 form to what was submitted to them to ensure it is correct.

I have Premium Turbo Tax 2024 desktop version. Part 5 of Schedule 3 is not pulling in data within Amount A and Amount B from Part 4. Why is this the case?
