Net conversions to a Roth IRA?

I converted an IRA prior to 2013 and made a withdrawal from the  Roth IRA in 2018. Turbotax is asking me "Report net conversions to Jame's Roth IRAs that were taxable and nontaxable in 2013 and prior years."

I don't remember the amount or the year. I just converted my IRA to a Roth way back when. What should I do?

EDITED: I am 71. I converted an IRA to a Roth IRA. I never made any other contributions to the Roth. I do NOT know when I converted the IRA to the Roth IRA. I was sometime between 1995 and 2013. I'd guess it was the mid-2000s, but I really don't remember.

Net conversions to a Roth IRA?

If you are under age 59½ and made no other contributions to Roth IRAs, you must find and enter the amount that was converted.  You can find this from the Form 8606 included with your 2013 tax return.  If you don't have a copy of your 2013 tax return, this will be in the transcript of your 2013 tax return that you can obtain from the IRS:
