My wife took an RMD this year in October 2017 from Allstate Annuity. Should they have sent us a 1099-R or some form to report income?

In most cases, yes, you should have received a 1099-R.   The payer should have sent it by Jan 31, 2018.  Call the issuer  to get a copy if it got lost in the mail.
