If I took a hardship dispersement from my 401k, can I still use the simple form?

I had federal and state taxes withheld, but have not received a 1099R.

What product should I use?

It depends.

If you have not reached age 59.5, you will be subject to a 10% penalty of the early distribution and need to file a Form 5329.  Therefore, you will need the TurboTax Deluxe edition. 

If you have reached age 59.5, there will be no penalty.  You can stay in the free edition.
