Box 11 reports distributions from non-qualified plans (like a 457(b) plan) that are included in box 1 and have nothing to do with a 401(k).
A distribution from a 401(k) would be reported to you on a 1099-R.

I do not think I am ever enrolled to a 457(b) plan. The only retirement related plan I was enrolled is a 401(k), and it is plain, no Roth or anything else. Other than 401(k), I also received some RSUs. I was expecting (salary + RSU + early withdrawal from 401(k)) would sum up to the value in Box-1 but there is huge difference (about 30%). It seems to me that either early withdrawal or RSU's are not included in my W-2. Is this possible?
I also did not receive a 1099-R. How can I get that form? My ex-employer was working with Fidelity. Is it Fidelity's responsibility to send me a 1099-R?
