Yes - Form 4852 can be used as a substitute 1099-R as well as a substitute W-2.
Please follow these steps to create a substitute 1099-R form and Form 4852:
- Click on Federal > Wages
& Income.
- Scroll down to the Retirement
Plans and Social Security section and click on the Start/Revisit box
next to IRA, 4701(k), Pension Plan Withdrawals (1099-R).

If you haven't already entered a
1099-R, you will see the screen, Your1099-R. Click Yes and
then Continue.
- If you've already entered one (or
more) 1099-Rs, you will see a screen Your 1099-R Entries. Click
on the link Add Another 1099-R (below the table).
- On the next screen click I'll
type it myself.
- On the screen, Tell Us Which
1099-R You Have, mark the radio button for the type of 1099-R.
- On that same screen,click the box next to I need to prepare a substitute 1099-r.
