I got the distributions from the bank but they didn't set it as 1099-SA form and they sent it as a statement. how should I enter the information into turbo tax and should I insert it as total or per transaction

You normally receive one, or at the most two 1099-SA forms in a tax season. More than two are possible but unusual.
You would like to know what the "bank" sent to the IRS (many forms are copied to the IRS), so I would do the following:
1. Look on the bank's website to see if there is a spot for tax forms, and if so, go look for 2025 1099-SA(s).
2. If no joy on the website, then call their customer service and ask why they didn't send you a 1099-SA as required by the IRS. See the Instructions for 1099-SA and 5498-SA where it says on page 1 "File Form 1099-SA, Distributions From an HSA, Archer MSA, or Medicare Advantage MSA, to report distributions made from a health savings account (HSA)..." and on page 2 "If you are required to file Form 1099-SA, you must provide a statement to the recipient."
Understand that these instructions are instructions to your HSA custodian, not to you. So the first quote tells the HSA custodian to send the IRS the 1099-SA(s), and the second quote tells the HSA custodian to send you a copy of what they sent to the IRS.
