I have a form 5498 from an inherited IRA. Where is this reported? It is NOT a 1099-R.

The form 5498 only lists #5, fair market value and #11, the required   minimum distribution.

I have a form 5498 from an inherited IRA. Where is this reported? It is NOT a 1099-R.

Nowhere -  the 5498 does not go on your tax return.  The 1099-R that you should receive does.   1099-R's must be sent by Jan 31.  If not received in a week or two after than then contact the account custodian.
