I filled out 4852 forms for three overseas pensions, only two printed out. Where is the third one? All the info for it is showing in Turbo Tax

When I go back into Turbo Tax the info for all three pensions is in there but when I print out my return there are only two 4852 forms in there.

I entered pension from a 1099-R. I entered rebate from a 1099-MISC. In my Form 1040, my pension shows twice, 5b and 8 (rebate+pension). How to remove pension from line 8?
