Roth IRA contributions are not specifically reported on your tax return, so there will not be a way to verify that you included them by looking at the pages of your return. However, if you can print your entire return, including all worksheets then you would find the contribution reported on the IRA Contribution Worksheet. This is something that is provided for your records, but not sent to the IRS as part of your return.
Since it is not required to report contributions to a Roth IRA on your tax return, an amended return would not be necessary.
In the event that you find the IRA Contribution Worksheet and no Roth contribution was recorded, you should take a look at the income limitations for contributing to a Roth IRA to be sure that a contribution is allowed. If you did not go through that section of your return, then the limitations may not have been applied.
