I did not ask for a 401K distribution
The company sent it to me because total amount was under their declared amount to keep it in, but I did not request a distribution.

Regardless of whether you asked for it or not, it must be entered on your tax return. You could have avoided taxes by rolling it over to an IRA within 60 days.
