Form 5329

I received a 1099-R for Distribution of Excess Contribution of ROTH IRA.  I entered the information from 1099-R into Turbo Tax but it did not generate a 5329. How do I create a Form 5329?

In what year was the Roth IRA contribution made?

For what year was the Roth IRA contribution made?

What is the year of the Form 1099-R provided by the payer?

What is the code in box 7 of this Form 1099-R?

Is the amount in box 2a of this Form 1099-R nonzero?

I figured it out, thanks.  But to answer your questions it was a contribution made in 2018 for tax year 2017.  A distribution was taken later in 2018 to correct the ineligible contribution, therefore the 1099R was for 2018.  

The 1099 R checked box 2b, and code P under #7.

 

I finally got Turbo Tax to create 5329 by backing into it in several ways.  One was to enter the tax manually on the 1040, then follow back to the worksheet that would populate that line and somehow fooling with it until I got the form to appear.

Form 5329

This was entirely reportable on your 2017 tax return.  Done properly on your 2017 tax return there would have been no excess carried to a 2018, so no 2018 Form 5329.  The only amount that would have appeared on your 2017 Form 5329 is the amount shown in box 2a of the Form 1099-R if there was a code 1 in box 7 in addition to the code P.
