Form 1099-R box 13 and box 14 are empty. Do I fill out the state information when entering the 1099-R information? (IRA converted to a Roth)

Box 13 is state payer's number and box 14 is state distribution.

No. When entering information from a Form W-2 or any form in the 1099 series, only enter information as it appears on your form. Do not enter zero where a box is left blank. Do not "correct" a form by entering information that you think ought to be there.

No. When entering information from a Form W-2 or any form in the 1099 series, only enter information as it appears on your form. Do not enter zero where a box is left blank. Do not "correct" a form by entering information that you think ought to be there.
