For Form 8606, line 6, do I include the value of Rollover IRAs and 401(k)s when determining the value of all traditional IRAs?

I'm still employed at the company where I have the 401K.

Rollover IRAs yes, 401(k)s no.  Only the total year end balance in traditional IRAs is to be included on Form 8606 line 6 (including SEP, SIMPLE and rollover IRAs).  A 401(k) is not an IRA.
