Does anyone have any experience with IRS form 5329?

If you missed the Dec 31 deadline, and or the April 1 deadline, you have to file Form 5329.

Unless improvements have been made, forum users complain it is hard to get Form 5329 for this situation using the Online version of Turbotax.
