Can I submit either a 1040EZ or 1040A form when making under $30,000.00 in 2017 and have a 1099-R form for withdrawing $5000.00 from my 401K? I do not itemized.

You can use the 1040A if the $30,000 you made in 2017 was only from wages, salaries, tips, interest, ordinary dividends, capital gain distributions, taxable scholarships and fellowship grants, pensions, annuities, IRAs, unemployment compensation, Alaska Permanent Fund dividends, and taxable social security or railroad retirement benefits.
If the $30,000 was for self-employment income you have to use TurboTax Online Self-Employed or TurboTax CD/Download Deluxe or higher.
