Can you clarify what the question actually is regarding your 1099-R?

It's two codes, 4 and D. Select code 4 in the first box 7 selection box and code D in the second box 7 selection box. In the online version of TurboTax you'll need to mark the box that says that you have a second code to enter.

If it says to enter box 14 and box 16 for state info, but my 1099r doesn't have a box 16? I used the same amount in both box14 and16 but it rejects it. Workaround?
