Box 2a is marked as UNKNOWN, Box 5 says None, and Box 9b is blank. (There is no 9a on my form.) Box 7 is "7". 

There has been federal income tax withheld (an amount listed in Box 4) and state tax withheld (an amount listed in Box 14). 

Do I leave Box 2a blank on my taxes, put down the full amount listed in Box 1, or something else altogether? 

For additional context, this is a monthly payment received as part of my former spouse's retirement, per divorce decree.
