I have a 1099R that is reporting a taxable gain on a distribution from a paid up life insurance policy  where do i put this  if i put it along with the other 1099R incomes it wants me to say what kind of retirement plan this is and its a dividend not a retirement distribution

Yes, but it is reported on a 1099R. After the 1099 R is reported, there is a question aking if this is a qualified or non-qualified plan. Indicates that it's non-qualified.
