1099 R form not entered?

I did not put in my 1099 form, what do I do now?

It said if I did not take any money out of my 401K I did not need to submit, but I think I did not do it correctly

Yes; if you have already filed, and received a 1099-R that you did not include, you need to amend.  Here are the instructions for amending your tax return.
