1099-r for roth

are there separate entries for a Roth cash out and my pension pay?  Both are on a 1099-r  Do I add them together or enter them separately on form?

Enter each 1099-R separately then continue thru ALL the follow up screens to answer the required questions ... do not skip about or you will miss them
