Filing Oklahoma NR form 500-B

How do I wire Oklahoma NR form 500-B if none of your associates/experts I spoke with know what it is, and it is not in your data base?  I need to wire this verification of withholding form to get my refund!

The instructions for NR Form 500B do not provide specific instructions for how to provide your copy of this form with your tax return. However, here is the mailing address for individual tax returns, which should be the same.
