form 8995 won't delete

I am not eligible for a Qual Business Income Deduction, but the form won't delete, and the worksheet won't either.

 

How can I get rid of it? I've tried the 'delete form' button -- doesn't help.

    Thanks for your time and effort. I'd already seen that, followed all the advice, and none of it worked.

    To remove the form you have to return to the place in the program that is generating the form.  What form is this coming from ?
