For tax purposes you do not need your health insurance subscriber number.  If you received a form 1095-B or 1095-C no information is required to be on your tax return.  If you did not have health insurance in 2019, no information is required to be on your tax return.

You only need to report if you had health insurance through the marketplace on your tax return.  If you have health insurance through the marketplace you should have a form 1095-A.  If you have this form this information needs to be put into your return for the tax return to calculate properly.

The information should be on your 1095-A. Please contact the issuer for the missing information.  Do you have something else? The 1095-B and 1095-C are no longer required to be entered. If you have a state plan that is required, it should have your personal ID number.

Your personal information may be where you enrolled in the plan and were assigned a number.
