DOES ANYONE KNOW WHERE THE STATE ID NUMBER IS LISTED IN REFERENCE TO NYS W-2 BOX 15?

It would be on your W-2 if there was state withholding in box 17. That tells you what state the withholding was for.

If there is box 17 withholding then contact the W-2 issuer for a corrected W-2.

If there is no state identification number in Box 15 of your W-2, please use the employer identification number (EIN) in box b as the state identification number.
