I worked for Shipt INC as a independent contractor. Should I place Shipt as business name?
The 1099-NEC for that i got has an account number. Should I place that as EIN?

No, your name is the business name and the business address is your address if you don't have a separate business location. Likewise, if you do not have an EIN then do not enter one at all. You are not required to have one unless you have employees. TurboTax will help you choose a business code, but if you don't find one that you feel represents your activity you can use 999999.
