What do I send in with my state tax payment (Alabama)

Once you have completed your tax return for Alabama, either by selecting to electronically file or file by mail, if there is a balance due showing on the Alabama tax return then there should also be a Form 40V showing with your printed return. This is Alabama's Individual Income Tax payment voucher which you will include with your payment. Please mail the Form 40V and payment via the address provided on the form.
