Are the 1099-SA and 5498-SA forms the same thing?

No, 1099-SA reports your distributions from your health savings account which must be used for qualified medical expenses. While the 5498 is a summary of your contributions for the year. You usually do not have to report anything from this form because your contributions are reported in Box 12/Code W on your W-2.
