With no place to enter this tuition expense (there is no 1098T) he is responsible for taxes and penalties on the earnings portion of the distribution.

You are able to enter education expenses without a 1098-T under 'Expenses and Scholarships' in the Education section (see screenshot).
Enter the 1099-Q distribution first, then go through the Education Expenses section. Only the earnings could be fully or partly taxable, as long as expenses exceed the 529 distribution.
