We have two Form 1098 from different lenders. Both forms have the same amount in Box 6 for Points paid on purchase. Do we include this amount for both lenders?

It depends. You should report only the points you paid, and not enter the total amount twice. 

But if you paid the same amount of points to both lenders, you would report each amount from Form 1098 as it appears.
