I am doing a tax return for my nephew, who passed away last year, and was a full time student. Do I need to enter 1098-T info even if the school refunded his tuition?

No, if they refunded his tuition you will not have to enter the 1098-T.  I cannot tell you how deeply sorry I am for your loss, please accept my condolence.
