You can not include a 2015 form in your 2017 tax returns. The IRS requires each year's tax return to be on a year by year basis.

If you did not file for 2015, you will need to gather this form and any additional documents from 2015 and file a prior return by mail (the IRS does not allow prior year returns to be e-filed).
