Schedule C

My wife worked proctoring exams for various institutions and received 1099 Misc's  for the income. We have had to fill out this schedule c in years past. She doesn't have a business name and this form is confusing me. It is asking for a business code and tax payer Id plus business address. what do i do with this??

You can simply use your wife's name as the "business name", your address as the business, and her social security number as the taxpayer ID number. 

For the business code, you can use something like 611000 or 999999 or you can select one from the list at the IRS site linked to below.
